
Does L-Citrulline Help Erections or ED? Clinical Research and Differences from Arginine
What clinical studies of men with mild ED say about L-citrulline, how it differs from arginine, and where the evidence remains limited.

L-citrulline is converted into L-arginine in the body and connects to nitric oxide pathways. That is why its relationship with erectile function has been studied.
There are not many clinical studies of L-citrulline alone in men with ED. It can look highly effective in search results, so it is important to look at the size and design of the studies.
A study included 24 men with mild ED
A 2011 clinical trial included 24 men with mild ED.
After one month on placebo, participants took L-citrulline for one month. The Erection Hardness Score has four levels. The number of men who improved from a score of 3 to 4 was 2 of 24 during the placebo period and 12 of 24 during the L-citrulline period.
The number of sexual encounters also increased during the study.
The difference looks large when shown as percentages, but there were only 24 participants. It was a small single-blind study limited to men with mild ED.
It did not establish the same result for moderate or severe ED, or show that an effect would remain after long-term use.

A small study also combined it with a PDE5 inhibitor
In 2018, a randomized double-blind crossover trial studied a combination of L-citrulline and trans-resveratrol in men who still had ED despite using a PDE5 inhibitor as needed.
Twenty men enrolled and 13 completed the trial.
The active period improved SHIM scores compared with baseline and placebo. There was no clear difference in the Erection Hardness Score.
This was not a study of L-citrulline alone. Participants also took trans-resveratrol and continued using a PDE5 inhibitor.
It is not possible to conclude that citrulline alone produces the same change.
How is it different from arginine?
L-arginine is a substrate in the nitric-oxide pathway. L-citrulline is converted into L-arginine in the body.
A 2019 review summarized evidence that oral citrulline may raise blood L-arginine levels more easily than oral arginine, likely because it uses a different metabolic route.
But raising blood arginine more easily is not the same as having a stronger effect on ED.
There is not enough large, direct comparison data in men with ED to determine whether L-citrulline or L-arginine improves erectile function more.

Is it an established ED treatment?
The EAU ED guideline gives L-arginine a weak recommendation for men with mild ED who do not want drug treatment. L-citrulline is not included in the same recommendation wording.
A 2025 review of amino-acid treatments found potential for L-arginine and L-citrulline but emphasized the need for larger studies of safety, dose, and combination with existing treatment.
L-citrulline is therefore not at the stage where it should replace a PDE5 inhibitor.
If ED continues, speak with a healthcare professional about evaluation and established treatment instead of watching indefinitely with a supplement alone.
The research does not establish a “right time before sex”
Some people want to know how many minutes before sex they should take citrulline.
The representative ED studies did not evaluate one dose immediately before sex. They assessed intake after a continued period.
It is not possible to determine an immediate-use schedule such as “30 minutes before” or “one hour before” from these studies.
