
Can ED Medicines Be Taken with Supplements? Cautions for Arginine and Citrulline
What to consider when combining ED medicines such as sildenafil with L-arginine, L-citrulline, or sexual-health supplements, including low blood pressure and drug interactions.

When using an ED medicine such as Viagra or Cialis, it is easy to wonder whether adding arginine or citrulline might make the effect stronger.
Clinical studies have in fact examined combinations of PDE5 inhibitors with some supplements.
But a combination studied in a trial is not the same as adding several over-the-counter supplements on your own. ED medicines and some supplements can both affect blood pressure, so the combination needs care.
Studies have combined ED medicines with L-arginine
The EAU ED guideline summarizes studies that combine PDE5 inhibitors with antioxidant-related ingredients and other supplements.
Trials adding L-arginine, folic acid, carnitine compounds, or resveratrol to PDE5 inhibitors have produced analyses showing better erectile-function scores than PDE5 inhibitors alone.
The ingredients and participants differed from one study to another.
This is not clinical evidence that combining an ED medicine with arginine will produce a large benefit for everyone. The EAU guideline also notes that improvements from supplements alone or alongside PDE5 inhibitors may be limited and that more high-quality research is needed to determine their clinical significance.
L-arginine can lower blood pressure
L-arginine is involved in nitric oxide pathways and can act in a direction that widens blood vessels.
The Mayo Clinic explains that L-arginine may lower blood pressure and that combining it with antihypertensive medicines can lower blood pressure too much.
It also cautions that combining L-arginine with sildenafil may lower blood pressure excessively.
If blood pressure falls enough to cause dizziness, lightheadedness, or fainting, this is no longer a minor attempt to add an erection benefit.
If you use an ED medicine or blood-pressure medicine and want to start L-arginine, checking with a doctor or pharmacist is safer.
L-citrulline is not automatically risk-free
L-citrulline is converted into L-arginine in the body and connects to nitric oxide pathways.
Small studies have added L-citrulline or similar ingredients for men who still had ED while using a PDE5 inhibitor.
There is less detailed clinical interaction data for L-citrulline and ED medicines than for L-arginine.
Do not translate limited data into “safety has been confirmed.”
Ask a healthcare professional if you take blood-pressure medicine, already have low blood pressure, or use several supplements.
Nitrates and ED medicines are an especially important combination to avoid
Before considering supplements, there is a combination that must be avoided with ED medicines.
The EAU guideline lists combining PDE5 inhibitors with organic nitrates such as nitroglycerin or nitric-oxide donor medicines as an absolute contraindication.
The combination can cause an unpredictable and severe drop in blood pressure. This includes nitrite drugs sometimes called “poppers.”
If you use nitrates for angina or another condition, do not use an ED medicine on your own.
This point takes priority over questions about arginine supplements.
What if you take an antihypertensive?
PDE5 inhibitors can cause a small additional drop in blood pressure when taken with common antihypertensives.
The EAU guideline states that this fall is usually small and that using several antihypertensives does not automatically create a major increase in PDE5-inhibitor side effects.
Some medicines that affect blood pressure, including alpha-blockers, still require care with the combination.
It is not true that everyone who takes blood-pressure medicine is unable to use an ED medicine. It is also not safe to combine them without checking the specific medicines.
Some “sexual-health supplements” contain undeclared drug ingredients
Ingredient labeling is another concern with over-the-counter products marketed to men.
The U.S. FDA has repeatedly published cases in which products marketed for sexual enhancement contained undeclared sildenafil, tadalafil, or similar ingredients.
Someone may believe they are taking only a natural supplement while actually taking something close to a PDE5 inhibitor.
For a person using nitrates, such a product can cause a dangerous drop in blood pressure.
“It is an imported sexual-health supplement, not a medicine” is not a safe assumption.
