Does Vitamin D Help ED and Erections? Deficiency and a Large Randomized Trial

We separate associations between vitamin D deficiency and ED from evidence that supplementation improves erectile function, including a trial of 8,920 men.

After reading that men with low vitamin D have more ED, you may wonder whether a supplement will also improve erections.

Observational studies and meta-analyses have found an association between low vitamin D and ED.

An association is not evidence that replacing vitamin D improves ED. A large randomized controlled trial published in 2024 did not find improvement in ED from vitamin D supplementation.

Studies have linked low vitamin D with ED

A 2023 review summarized studies of vitamin D and ED.

Earlier meta-analyses reported lower vitamin D levels among men with ED and an association between vitamin D deficiency and lower IIEF-5 scores.

One proposed mechanism involves the vascular endothelium. Vitamin D deficiency may relate to vascular function, inflammation, and nitric oxide.

Age, obesity, diabetes, hypertension, and inactivity are also shared factors for low vitamin D and ED.

Observational data cannot easily separate whether low vitamin D caused ED or another health condition contributed to both.

A trial of 8,920 men found no ED improvement

The 2024 analysis of the D-Health Trial is important for this question.

It compared three years of vitamin D supplementation with placebo in 8,920 men aged 60 to 84.

Blood vitamin D levels rose clearly in the supplement group. ED prevalence was still 58.8% with vitamin D and 59.0% with placebo.

The prevalence ratio was 1.00, and no difference from supplementation was confirmed. Subgroup analyses did not show improvement either.

At least in this large trial, adding vitamin D did not prevent or improve ED.

Treating a deficiency is a separate matter

The trial does not mean that confirmed vitamin D deficiency should be ignored.

Vitamin D is relevant to bone and other health, and a medically confirmed deficiency may require replacement.

The treatment goal should be separated from “making erections firmer.”

Improving overall health after treating deficiency is possible, but vitamin D cannot be called an ED treatment on that basis.

Will it raise testosterone too?

Vitamin D and testosterone have also been studied together.

Observational studies have found associations between low vitamin D and low testosterone, but a meta-analysis of randomized trials of vitamin D supplementation did not confirm a clear testosterone increase.

The straight line “take vitamin D, raise testosterone, improve erections” is not supported strongly enough.

If low testosterone is suspected, evaluate it with blood testing rather than guessing from supplements.

The EAU treats vitamin D deficiency as one associated factor

The EAU ED guideline lists vitamin D deficiency among factors reported to be associated with ED.

It also treats ED as a condition in which vascular, nerve, hormonal, psychological, diabetes, hypertension, and inactivity factors can overlap.

Checking vitamin D alone cannot identify the cause.

If changes in erections continue, a healthcare professional can decide whether blood sugar, lipids, blood pressure, testosterone, and other tests are appropriate.
