
Can Smoking Cause ED? Smoking, Vaping, and Erectile Function
What research says about smoking and ED, changes after quitting, and what is known and not known about e-cigarettes and vaping.

It can be difficult to see how a lung-related habit connects to sexual function, even when you hear that smoking makes erections harder.
Blood vessels play a major role in erections. Smoking is a cardiovascular risk factor, and multiple studies have examined its association with ED.
Current research associates cigarette smoking with higher ED risk. E-cigarettes and vaping also have some association data, but the long-term research is not as established as it is for cigarettes.

Smokers had higher ED risk in research
A 2013 meta-analysis examined four prospective cohort studies and four case-control studies involving 28,586 people.
Across the prospective cohort studies, current smokers had 1.51 times the odds of ED compared with non-smokers. Former smokers had 1.29 times the odds.
Another dose-response meta-analysis combined 10 studies and 50,360 people and reported a 1.14-times higher ED odds for each additional 10 cigarettes smoked per day.
These are mainly observational results, so they cannot prove that smoking alone caused ED in one person. Still, several studies pointed in the same direction for smoking amount and ED.
Why are smoking and erections connected?
An erection requires blood vessels to widen and deliver blood to the penis.
A 2015 systematic review described vascular endothelial damage, reduced nitric-oxide availability, and oxidative stress as possible links between smoking and ED.
The EAU ED guideline also lists smoking as a known ED risk factor. It can overlap with other vascular risks such as inactivity, obesity, high blood pressure, and diabetes.
Consider sexual function alongside vascular and metabolic health rather than as an isolated local problem.
Can erections improve after quitting?
Some research supports improvement after quitting.
A 2004 prospective study followed smokers with ED while excluding other major risk factors and compared people who quit with those who continued. Erectile function improved in the group that quit.
A 2015 systematic review also included studies supporting a possible recovery of erectile function after smoking cessation.
This is not a guarantee that quitting will completely reverse years of smoking. The result differs with smoking history, age, vascular condition, diabetes, hypertension, and other factors.
Is vaping unrelated?
The evidence is less settled than for cigarettes.
A 2021 cross-sectional study using the U.S. PATH Study found that men who used electronic nicotine-delivery systems daily had a higher reported prevalence of ED than men who had never used them.
Because it was cross-sectional, the study did not prove that vaping caused ED.
A 2026 review also examined possible effects of e-cigarettes on endothelial function, nitric oxide, and oxidative stress, while noting that long-term prospective research is lacking and causation and dose-response remain unestablished.
It is not safe to say that vaping has no sexual effect simply because it is not a cigarette. It is also too early to say that vaping inevitably causes ED.
