What the EAU guideline and 2026 ICSM recommendations say about testosterone, penile growth before puberty, and adult enlargement.

Growth before puberty and increasing the size of an adult penis after development are different questions. The 2026 ICSM recommendations and the EAU guideline do not recommend testosterone therapy for adult penile enlargement.

Testosterone does play a role in penile development

The penis develops under hormonal influence during fetal life and puberty.

For limited medical conditions such as true micropenis or differences of sex development before puberty, hormones such as testosterone or DHT may be used.

The EAU states that hormone treatment can restore penile size in these pediatric situations.

It is tempting to extend that fact to adults, but the timing and physical condition are different.

Testosterone does not enlarge the adult penis

The EAU guideline on penile size concludes that testosterone therapy does not increase penile size in adult men or men with age-related hypogonadism.

It strongly recommends against using testosterone or other hormone treatment for adult penile enlargement after puberty.

The 2026 ICSM recommendations point in the same direction.

They limit hormone therapy as an option to conditions such as true micropenis or differences of sex development before puberty and strongly recommend against testosterone for enlargement after puberty.

This is different from treating confirmed low testosterone

When an adult man has confirmed low testosterone along with reduced libido or ED, medical testosterone treatment may be considered.

The goal is treating symptoms of hypogonadism.

Studies in men with low testosterone show improvement in libido and some sexual functions, but not that the penis itself becomes longer.

Separate “erections became firmer and look larger than before” from an increase in tissue length.

Does a temporary rise after strength training change size?

Studies can find a temporary change in blood testosterone after strength exercise.

There is no clinical evidence that this short hormone change grows adult penile tissue.

Squats, lunges, and planks should not be presented as exercises that enlarge the penis.

The same applies to “testosterone boosters”

Some supplements say “testosterone support” or “men’s strength” on the label.

Even if correcting a nutrient deficiency changes testosterone in a particular case, there is no clinical evidence that this enlarges an adult penis.

Research on zinc, vitamin D, or maca for erections or libido is also different from permanent penile enlargement.

Adding several supplements does not restart post-pubertal penile growth.

Measure and assess the situation before chasing size

The ICSM recommends appropriate measurement and distinguishing true micropenis, acquired buried penis, and severe anxiety about penile size in people who are concerned.

Weight gain can hide the base of the penis in surrounding fat and make visible length appear shorter in adults.

Testosterone is not the solution in that situation either.

If size concern is strong, speak with a urologist before using enlargement supplements or hormones on your own.
