
Do Clamping and Hanging Work? Why to Avoid Them for Penis Enlargement
We review the evidence and injury risks around clamping and hanging, and explain why they should not be confused with medical traction or ED devices.

Clamping is described as strongly constricting the penis, while hanging is described as applying strong traction with weights or similar equipment.
Search terms may include “penis clamping,” “clamp,” “hanging,” “weighted hanging,” or descriptions such as hanging weights from the penis. Different labels do not make self-directed strong constriction or weighted pulling the same as medically managed devices.

There is no established clinical evidence that clamping permanently increases girth
Clamping is sometimes explained as a way to make tissue grow by trapping blood and creating high pressure.
There is no established high-quality clinical trial showing that this method safely and permanently increases penile circumference in healthy adult men.
Vacuum Erection Devices used in medical care may use a ring when necessary to maintain an erection.
That does not mean the device can be converted into self-directed clamping by “squeezing harder” to create enlargement.

Strong constriction can become a penile-strangulation emergency
When a ring or other object becomes embedded around the penis and obstructs blood flow, penile strangulation can require emergency urologic treatment.
A 2007 review reported a range of injuries from mild circulatory problems to severe ischemia and gangrene.
A 2008 review also reported more serious injuries in cases where constriction lasted longer than 72 hours.
A 2024 review examined 100 cases reported from 2000 to 2019 and reported lasting problems after release in 24 cases.
In 2026, a case was also reported in which a penile ring could not be removed for 19 hours and the patient presented unable to urinate.
The fact that pain feels tolerable does not prove that the situation is safe.
Hanging is not the same as medical traction
Penile Traction Therapy, which pulls the penis in a controlled direction, has been studied clinically.
The 2026 ICSM recommendations summarize studies of dedicated traction devices showing limited length increases.
That is not evidence supporting self-directed hanging with weights.
The dedicated devices used in research control how force is applied within a defined range.
Those studies are not efficacy trials of continually increasing weight or pulling until pain appears.
It is not medically valid to jump from “dedicated traction has some research in a clinical context” to “self-directed hanging will produce the same result.”
More force does not mean more size
Strength training sometimes uses progressive loading.
There is no basis for applying that idea to penile enlargement by “clamping harder” or “using heavier weights.”
Injury can cause swelling, bruising, reduced sensation, and damage to skin or blood vessels. It may also affect erectile function later.
If the goal of changing size damages sexual function, the result is the opposite of the goal.
Clamping and an ED-treatment ring are not the same thing
A constriction ring used with a VED can have a medical purpose: temporarily maintaining an erection created with negative pressure.
Even medical use can involve pain, numbness, or bruising and is not intended for prolonged use.
Its existence does not make strong constriction for enlargement safe.
Even if internet slang uses similar names, separate the original purpose, the safety of the equipment, the conditions of use, and whether a claim is supported by scientific research.
Seek help quickly if it will not come off, changes color, or reduces sensation
If a constricting device will not come off, the penis is markedly swollen, the color turns purple or dark, sensation is reduced, or urination becomes difficult, this is not a situation for watching at home.
With penile strangulation, prompt release is needed to reduce tissue injury.
